Physical Examination for Sophomores:

 
 Dear Parent or Guardian of the incoming Sophomore class:

     The Mehlville School District recommends that children have a complete physical examination upon entrance into 10th grade (sophomore year).  If a physical exam has been completed within the past year and there are no medical concerns, a copy of this examination will be accepted.  This is in accordance with the recommendations of the St. Louis County Medical Society School Health Committee.  

      The Physical Examination Forms provided by your doctor’s office (MD), Doctor of Osteopathy (D.O.) or Nurse Practitioner (NP) will only be accepted.

      Please submit this documentation, along with an updated immunization record, to the MHS Nurses Office as soon as it has been completed.  State law requires that immunizations be up to date and completed prior to entrance to school.  There is no grace period allowed.

Physical forms and updated immunization records can be mailed to: MHS Nurse’s Office, 3200 Lemay Ferry Road, St. Louis, MO 63125, faxed to the attention “MHS Nurse’s Office” at 314-467-6099, emailed to [email protected]  or uploaded in the parent portal.

*Medical documents can be uploaded via the Parent Portal on the Mehlville School District website. Here you may submit an E-Form for physicals, immunizations, medication consent or action plans.
